Resource Library: Personalised Care and Support Planning

  • Personalised care and support planning (PCSP) is a series of facilitated conversations in which the person, or those who know them well, actively participates to explore the management of their health and well-being within the context of their whole life and family situation.

    PCSP is key for people receiving health and social care services. It is an essential tool to integrate the person’s experience of all the services they access so they have one joined-up plan that covers their health and wellbeing needs. The process recognises the person’s skills and strengths, as well as their experiences and the things that matter the most to them. It addresses the things that are not working in the person’s life and identifies outcomes or goals and actions to resolve these.


    • Our evidence-based infographic provides a quick overview of the importance and benefits of PCSP. For example, the pie chart below shows that 45% of people in H&W that responded to the GP patient survey in 2023 said that their plan was very helpful in managing their condition.

The Personalised Care Team at NHS Herefordshire and Worcestershire also offer virtual, trainer-led sessions on an ad-hoc basis, which can be tailored to specific service needs.         • PCSP e-Learning from PCI: This course from the Personalised Care Institute (PCI) provides a definition and overview of the criteria for a good PCSP and highlights through a range of case studies the key steps of the planning process. Time to complete: 45 minutes. 

        • PCSP Maternity e-learning from PCI: This course from the Personalised Care Institute (PCI) discusses how healthcare professionals can support women to make safe and informed decisions during their antenatal care and delivery. Time to complete: 30 minutes.
