Worcestershire (18+) Worcestershire Acute Hospitals NHS Trust Specialist Palliative Care Service
Our Specialist Palliative Care team provides advice and support for patients with advanced life-limiting illnesses, as well as for their families and carers. This is a service we offer seven days a week, with specialist nurses available to help patients with complex needs or uncontrolled symptoms from 8.30am to 4.30pm every weekend and every bank holiday.
Our team is based at the Alexandra Hospital, Redditch and Worcestershire Royal Hospital, and we can:
- Advice and management ofcomplex symptoms such as pain, nausea, vomiting, anxiety, breathlessness, and fatigue (overwhelming tiredness)
- Psychological support and advice to patients, carers and significant family members
- Liaison with nurses, doctors, physiotherapists, occupational therapists, chaplaincy, social services and other people involved in your care both in hospital and at home
- Liaison with local specialist palliative care services, hospices, day care services and community teams
- The opportunity to discuss your preferences and wishes to help you plan future care
- Advise on practical things, such as finances

Worcestershire (18+) Herefordshire and Worcestershire Health and Care NHS Trust Specialist Palliative Care Service
We provide the community SPC service for North Worcestershire and Tenbury, and also a 6 bedded Specialist inpatient unit “The Primrose unit” within the Princess of Wales Community Hospital in Bromsgrove.
We see patients with progressive, life limiting illnesses who have physical, psychological or social needs which their current care providers are struggling to optimise at the times when this is most needed.
We work closely with our Hospice and acute Trust partners.
Community SPC Team
The team comprises Clinical Nurse Specialists, a clinical psychologist, and Consultants in Palliative Medicine. We see patients registered with a Wyre Forest, Redditch and Bromsgrove, or Tenbury GP, usually in their own homes, but also as outpatients or remotely via video or telephone.

Worcestershire (18+) St Richard's Hospice
St Richard’s Hospice is here for anyone over the age of 18 in Worcestershire who lives with a serious, progressive illness.
Everything we do focuses on providing the best care for patients, their loved ones and carers at home, at the hospice or via support groups.
Our patients are so much more than their illness and with our support they can make the most of every day.
Our care in the community
Our knowledgeable and experienced Community Nursing Team offers a specialist service to help patients and families manage their care in their own surroundings.
Our care at the hospice
Our Living Well Centre offers clinical and wellbeing support in a friendly space. Our team empower patients and families to live as well as they can. Wellbeing programmes include Tai Chi and creative therapy.
Our home-from-home In-patient Unit provides 24-hour care tailored to each person’s needs. Our comfortable and cosy rooms have en-suite facilities and a private patio area overlooking the lake and gardens.
Our Family Support Service gives emotional and practical support for adults and children including counselling and group sessions. We provide a safe, confidential place to talk where concerns are heard in a non-judgemental and caring way.

Worcestershire (18+) Primrose Hospice
The Primrose Hospice offers care and support for adults living with a life limiting illness and we are here to ensure patients are able to live your life as fully as possible.
We offer a Day Hospice that offers clinical support and understanding in a relaxed environment. We aim to empower patients and families to make decisions which best suit needs and diagnosis and to support our patients to live as well as they can.

Wellbeing service
Our wellbeing services are here to help manage and maintain your quality of life. This includes activities such as:
- Men’s Group – Incorporates Days Out
- GALS Group – Informal support group for ladies
Family Support Services
We provide emotional support through individual counselling and group support. We support adults and children who are bereaved or have someone they are close to with a life limiting illness. Family support can also help with benefits advice by talking with other agencies and ensuring views are heard.
The Clinical care & Wellbeing Teams work closely with the Family Support Team to ensure we are offering a holistic approach to care and support.

Worcestershire (18+) Kemp Hospice
KEMP day hospice sits at the heart of the Wyre Forest, and we’re open to anyone over the age of 18 in the Wyre Forest district. It's a happy hub where patients, their families and carers receive the support they need from our team of specialists through one-to-one support, groups, and virtual sessions.
We work with each person to create a holistic, bespoke programme of care which is regularly reviewed and adapted to the individual's needs.
Spending time at our hospice can include:
- Nursing care – holistic assessment and individualised support.
- Exercise and Wellbeing groups face to face or virtual and individual support.
- Occupational therapy, helping to maintain independence and function.
- Creative Activities and groups.
- Finance & Welfare Support.
- Psychological Support and counselling for patient and loved ones.
- Our inhouse chef caters for all diets and individual needs and cooks up a varied menu daily.
Our virtual day hospice and one-to-one support means patients who can’t visit the hospice can still get support they need from our team and other patients.
At times, the team will visit patients at home when they are unable to attend the hospice.

Herefordshire (18+) Wye Valley NHS Trust Specialist Palliative Care Service
The team comprises consultants in palliative medicine, specialist palliative care CNSs and psychologists whose focus is looking after people with a progressive, life limiting illness where symptoms whether physical, psychological, social or spiritual may be impacting on their quality of life. They work across all care settings including Hereford County Hospital, Herefordshire Community Hospitals and Herefordshire and mid Powys community, seeing people wherever is most appropriate for the individual person. The team work closely with colleagues at St Michaels Hospice.

Herefordshire (18+) St Michael's Hospice
Professional staff and trained volunteers are committed to improving the quality of life for our patients and their families and carers. We can offer Day Services (including specialist clinics), Inpatient Services, Hospice at Home Service and support for families and carers (including financial advice and practical support).
We care for cancer and non-cancer patients and, because everyone’s illness is unique, we offer a range of different treatments. Our outpatient clinics and support groups can give you the confidence and advice to help you live well with your illness.
Our person-centred approach means we also provide psychological and emotional support from our counselling and social care team. Visiting the Hospice offers you a chance to socialise, meet new friends and take part in activities ranging from arts and crafts to informative talks. Many of our services are also available for your family and carers.
Hospice at Home Service
For patients who wish to stay at home, we provide a Hospice at Home Service. Hospice care staff visit the home to offer support and discuss your care. Towards the final stages of your illness, we can arrange for hands-on nursing care in the home at times when the patient and family need it.
